Has this been submitted to Heaven so that others can use it?

It hasn't, though it could be submitted.  I just designed one myself out of needing one for a game that I started a little while ago.

Well, it looks like a good sheet, and Heaven doesn't seem to have one, so you should!

Very nice. Just a note for others out there who play on the go, this only seems to work well in the crime browser of my mobile devices. IPad and iPhone seem to be barfing as do the other browsers on the android.
