Posting Rules
The Rules below are DEPRECATED.  There are flaws in them.  For now, everyone is expected to simply be reasonable about their posting rate (this is still intended to move slowly) while making sure that no player is left behind.  Don't post too frequently. Give everyone a solid chance to respond.

Eventually, we will return to clear posting guidelines, but they need to be discussed and re-evaluated.

All strikes as a result of the rules are cleared (because if the rules are bad, the strikes are too).

Original post:

This game will have a MINIMUM posting rate of once per two weeks.  A player unable to stick to this limit gets two warnings.  After a third breach, the player will be removed from the game, and his character turned into an NPC or given to a new player.  Note: After receiving their first warning, they have only have one week before they get a second, then one more week before getting removed.  If the player posts after getting a warning, they are returned to the two week minimum.

This game will also have a MAXIMUM posting rate of once per week.  Player John is only allowed to post if a week has passed since his last post, unless all other players (and myself), have already posted since Player John's last post.  A player who breaks this rule gets two warnings.  After a third breach, the player will be removed from the game, and his character turned into an NPC or given to a new player.

If a player does not meet the minimum, a GM post will be made excluding them from the Maximum for each other player's next post.

These rules are intended to ensure that posting stays at a steady rate, while still giving people time to live their lives and think through their responses without excessive pressure to progress the game, while also allowing the game to move more quickly if ALL players are in a state where they feel able to do so.

The GM is exempted from the Maximum rule, in order to allow for rule judgements and other reactions to player posts that would affect the posts of other player.  If the GM excessively abuses this exemption, he should be called on it.

The GM is not exempted from the Minimum rule.  If the GM breaches this rule, he should be called on it.

Yes, a GM can't really be removed from a game in quite the same way as a player, but hopefully being called on breaches repeatedly will lead to the GM improving his behaviour, or arranging to hand the game off to another GM who can stick to the rule.
