Do we get a surprise round?
You whiny baby. There is no initiative and players always go first. If you think you need a surprise round then you probably don't have what it takes to kill the bad guy no matter what you do.

How do we handle secret passages? My passive perception should notice it but I don't wanna meta game.
Since you the player know it's there, make a perception roll to see if your character notices it. Passive perception is for weenies. Which is why its called PP.

Ha! Try killing someone and find out.

Arrow Slits?
1.) The arrow slit is big enough for a man to climb through, so about 2ft tall. You would have disadvantage if you were outside trying to attack the player inside that was adjacent to the slit but: It is possible to hit the player inside the fortress.
2.) Shooting out of an arrow slit from 5' away is at disadvantage.

Can we use the optional disarm rules from the DMG on page 271?
NO

Since PVE is theater of the mind, how do spell shapes work?
In everyone's best interest. For the most part you can line up a shot to effect at least 2 enemies. That's a 2 way street cowboy.

How does AOO work in TOTM?
To keep it simple, only when you've moved to melee an enemy can that enemy take an AOO on you (and vice versa), or if it has specifically been stated that an enemy is standing right next to you.

When a player quits...or gets turned into dust...can we keep their loot?
That’s why there is a loot thread. I’m not gonna keep track of that stuff. I just turn people to dust. It’s your job to clean up the dust. Or you can wait until Monday. That’s when the cleaning lady comes. She dusts.

FAQs
Do spell components get consumed?
If it doesn't say its consumed then no, you just wave it around and put it back in your pocket.

Material (M)
Casting some Spells requires particular Objects, specified in parentheses in the component entry. A character can use a Component pouch or a Spellcasting focus (found in “Equipment”) in place of the Components specified for a spell. But if a cost is indicated for a component, a character must have that specific component before he or she can cast the spell.

If a spell states that a material component is consumed by the spell, the caster must provide this component for each casting of the spell.
A spellcaster must have a hand free to access a spell’s material components—or to hold a Spellcasting focus—but it can be the same hand that he or she uses to perform somatic Components.
