Rolling initiative EVERY round! Is this a new idea? Or am I missing it in all of your other games!

It isn't a new idea, but appears that I've found the format to make it clear.

Basically, it is part of my theory that combat is a very chaotic (not in the alignment sense) situation.  I'll also be rolling the same dice for the foes, and then I have a word doc on the side here to help me sort it out.
