My ruling is that players should wait to cash in XP for PP's during Transition Scenes. I think being able to exchange them any time (which admittedly seems to be the gamebook's ruling) would tend to over-incentivize hitting that 1 XP milestone every turn, no matter what, and that could get old quick from a roleplaying perspective. If players can only exchange them in Transition Scenes, I also think it would give everyone more reason to look forward to those breaks in the action so they can restock their PP's.
Re: the second question, you have to roll the d4 in a dice pool first before you get the PP to use on your following turn. There's a good reason for this too-- otherwise, heroes with healing or invulnerability SFX that operate off PP's aren't just difficult to K.O. through physical stress-- they become impossible to defeat that way. If it helps to know this, most things in MHR operate off the logic of "use
(trigger) to get
(benefit)." In this case, rolling the dice pool containing the d4 is the trigger, and getting the PP is the benefit.
