Not to get too deep into just an idiom, but methinks it gets repeated and used for slightly different cases, until it ceases to have any significant meaning, much like any other idiom (e.g. 'boomer' had a very specific definition initially, and I've heard/read people use it for anything from 'old-fashioned' to 'you said comething uncool according to me'). I do think it initially held that meaning: someone who should cringe at their own words/behavior, but doesn't, so the observer/speaker feels the cringe for them. So, a degree of empathy involved indeed.
