In short, Birthright is a low-magic traditional fantasy world, in which the players can take on the role of divinely-empowered rulers, with an emphasis on the political rulership in the gameplay. The setting revolves around the concept of Bloodlines: divine power gained by heroes and passed to their descendants. Characters with a bloodline create an aura of command known as Regency, and can acquire a domain composed of provinces and/or holdings. The development of these domains is as much a part of the game as the traditional RPG development of the characters.
